Definition

A network is the group of doctors, hospitals, labs and pharmacies that have contracted with an insurer to accept negotiated rates. In-network care carries the plan's discounted pricing; out-of-network providers can bill their full charge, exposing patients to balance billing.

Before scheduling an MRI, a patient uses the insurer's provider directory to confirm the imaging center is in-network. The same MRI billed at $2,400 out-of-network is repriced to $640 in-network.

Why It Matters

Network status is the strongest single predictor of medical debt. Roughly two-thirds of surprise bills investigated by the CFPB stem from ancillary providers (anesthesiologists, radiologists) who were out-of-network at an in-network facility.
